Job Summary
Provide confidential assessments, individualized treatment plans, goal‑setting assistance, coaching, health education, and self‑care tools to clients in a contracted employee wellness program. Refer clients to other programs and work with Primary Care Physicians, Care Coordinators, and Health Management resources as needed.
Experience
Minimum 3 years inpatient or outpatient care with patient education (5 years preferred)
Experience in health and wellness coaching, patient advocacy, or case management preferred
Education
Associate’s degree in Nursing required
Bachelor’s degree in Nursing preferred
Training
Epic training within 90 days of hire required
Special Skills
Knowledge of HPIAA, ADA, and evidence‑based chronic conditions protocols
Excellent oral and written communication
Motivational interviewing strategies
Independent and autonomous work ability
Proficiency with Microsoft Office Suite and Publisher
Epic experience preferred
Smoking cessation counseling experience preferred
Health Risk Appraisal software experience preferred
Licensure
Current Wisconsin Registered Nurse License or multi‑state license under NLC
Valid Wisconsin driver’s license
American Heart Association Basic Life Support (BLS) or equivalent required
Certified Wellness Coach/Advocate preferred (Wellcoaches or NCHS)
